Please call our office at 910-353-4333 then press option 2 when prompted to request your prescription refill. Your request will be handled if received before 3PM the same day. If after 3PM, it will be processed the next business day. If this is an emergency please call our office for 'after' hours contact information or dial 911.

Requesting Medical Records

To request Medical Records please come by the office and fill out a release of information form. Due to the amount of patients we have in our practice it takes us 7 - 10 business days to process the request of Medical Records. We will mail records and fax (when available) to other providers free of charge. If you request records to be picked up there will be a fee of $.50 per page that is paid when you pick up the records. If you need to check the status of your Medical Records request please call (910) 353-4333 and press option 4 when prompted.
