PRIOR TO YOUR FIRST VISIT

• Please verify that the Women's Healthcare Associates physician/clinician you are scheduled to visit is ‘contracted’ with your insurance provider.
• If you are uninsured please make payment arrangements prior to your first visit.
• If you are transferring care to Women's Healthcare Associates, come into the office to sign a release of medical records form or have your previous provider fax your records to our facility. Our fax number is (910) 353-6529.

FIRST APPOINTMENT TO OUR OB/GYN OFFICE PLEASE BRING

• Your insurance card if you have health insurance
• Your completed Medical History form
• Your Completed Hippa Form
• A picture ID

HOURS & APPOINTMENTS

With the exception of urgent medical situations, we see patients by appointment only. Our hours are Monday – Friday 8:00 AM until 11:45 AM and 12:45 PM until 4:30 PM. You can schedule your appointment by calling (910) 353-4333.
