INSURANCE INFORMATION

We participate in most insurance plans that serve Jacksonville and surrounding areas. To verify participation with your insurance carrier, please contact your health plan or the patient account representative at our office (910) 353-4333.

We participate with most major insurance carriers including:

• Cigna Healthcare
• United Health Care
• Blue Cross Blue Shield
• Medicaid and Medicare

BILLING & PAYMENT

For our first time mother-to-be patients, we will schedule time with you at our office to review and discuss your insurance coverage and try to answer any questions or concerns you may have about your financial obligations during your pregnancy and related to labor and delivery.

For patients without insurance coverage- If you do not have insurance coverage or your insurance card with you, you will be expected to make financial arrangements, including a deposit on the day of your appointment and payment arrangements for long-term care or procedures.

Your co-pay and deductibles are due at time of service. A monthly statement will be mailed to you showing your balance after your insurance has paid. Your bill is due and payable within 30 days after the billing date. We accept cash, checks, mastercard and Visa.

Decoding Your Coverage:

Deductibles are paid out of pocket each year before insurance kicks in.  Each family member usually has a separate deductible.

Co Payments are flat fees charged each time you visit the doctor or use any medical service, regardless of the cost of the procedure.

Coinsurance requires you to pay a percentage of the total cost of the care.  Many plans require either co insurance or co pay but some charge both.

Maximum out of pocket expense is the most you’ll have to spend before all of your medical bills are covered.
